Intake Process
We prefer to help you fill out all paperwork prior to your first appointment, so when you come into the studio we get to know you and begin creating your personalized care plan. Feel free to download the paperwork below and send it in, and we’ll call to schedule your visit!
Your First Visit: What to Expect
New Patient Interview
Your interview takes place in a welcoming, art-filled meeting room. Have a cup of tea as we get to know you--as both a patient and a person--and discuss your medical and dental history, gathering information, in your own words, about your needs, goals, and prior healthcare experience. We also just want to learn a little bit about who you are!
Comprehensive Exam
Each new patient receives a comprehensive oral exam to establish a dental health baseline. The exam includes the following:
- Vital signs
- Head and neck (muscles, joints, lymph nodes, glands)
- Imaging (ie, x-rays) appropriate to the patient’s needs
- Occlusion (checking bite and measurements in static positions)
- Oral cancer exam
- Periodontal (gum and bone support: checking for pockets around teeth, gum recession, bone loss, tooth mobility)
- Dentition exam: tooth-by-tooth examination of each tooth’s condition and needs.
Patient Care Plan
After your initial exam, Dr. Jamie will suggest ways you can make immediate at-home improvement to your oral health, as well as discuss an in-office care and management plan. She will send you a detailed, personable slideshow with an after-visit summary and outlined treatment plan.
Insurance
We partner with our patients to help them understand how their dental benefits work, and can submit claims to most insurance companies on their behalf.
